TREE DEDICATION NOMINATION REQUEST

The Kiwanis Club of Middletown, in cooperation with the City of Middletown, is offering the opportunity to dedicate a tree in honor of, or in memory of, an individual whose actions or deeds have positively impacted the Middletown community. The tree shall be planted in Middletown and shall be identified by a dedication plaque which will name the honoree. Please fill out and submit the form below to make your nomination.
